A Rare Presentation of Thyrotoxicosis: Gynecomastia

Gynecomastia is the proliferation of the glandular breast tissue in men and often a benign course of breast growth. The main factor that led to the development of gynecomastia is deterioration of sex hormone balance due to increased production of estrogen or decreased levels of androgen. Therefore, physiological causes, endocrine dysfunctions, systemic diseases and medications should be considered that may affect the sex hormone levels in assessment of patients with gynecomastia. Despite gynecomastia has long been associated with thyrotoxicosis, the patients admitted with gynecomastia is quite rare and limited to case reports. This case report evaluating a 66-year-old male patient with bilateral gynecomastia and emphasized the need to be considered in the differential diagnosis of gynecomastia even thyrotoxicosis symptoms are not in the foreground. It is important to keep in mind to treat both the underlying pathology and gynecomastia.

Tirotoksikozun Nadir Bir Başvuru Sebebi: Jinekomasti

Jinekomasti, erkeklerde memedeki glanduler yapının proliferasyonu sonucu oluşan ve sıklıkla benign klinik seyirli meme büyümesidir. Jinekomasti gelişimine sebep olan ana faktör östrojen üretiminde artış veya androjen seviyesinde azalma ile seks hormon etkilerindeki balansın bozulmasıdır. Dolayısı ile jinekomastili hasta değerlendirmesinde fizyolojik sebepler, endokrin disfonksiyonlar, sistemik hastalıklar ve seks hormon düzeylerini etkileyebilecek ilaçlar göz önünde bulundurulmalıdır. Jinekomasti uzun zamandan beri tirotoksikozla ilişkilendirilmiş klinik bir durum olmasına rağmen jinekomasti ile başvuran tirotoksikoz vakası oldukça nadirdir ve vaka sunumlarıyla sınırlıdır. Bu vaka sunumunda, bilateral jinekomasti ile başvuran 66 yaşında bir erkek hasta değerlendirilerek, tirotoksikoz semptomları ön planda olmasa bile jinekomasti ayırıcı tanısında tirotoksikozun göz önüne alınması gerektiği vurgulanmıştır. Bu durumun akılda tutulması hem altta yatan patolojinin hem de jinekomastinin tedavisi için önem taşımaktadır.
