The Effects of Sirolimus and Takrolimus on Liver Regeneration

INTRODUCTION: In this study, the effects of sirolimus and tacrolimus on liver regeneration in rats with partial hepatectomy were compared in the absence or presence of the corticosteroids.
METHODS: Forty-eight male Sprague-Dawley rats (approximately 200-300 g) were randomly assigned to five groups. Each one underwent partial hepatectomy. 4 days after partial hepatectomy, sirolimus or tacrolimus in combination with or without corticosteroids (prednisolone) was intraperitoneally injected The rats were sacrificed at the end of four days, remnnat liver tissues were weighed and the weight index was created. Remnant liver tissues that stained with hematoxylin and eosin, were examined under a light microscope in terms of mitosis.
RESULTS: In the comparison of mitotic index and weight index, between the sirolimus group and tacrolimus group; sirolimus group had statistically significant effects. And between the sirolimus group and control group, there were no statiscally significant differences. But in the comparison of mitotic index and weight index, between the sirolimus+ steroids and control group, there were statitiscally significant differences.
DISCUSSION AND CONCLUSION: Investigating the effects of sirolimus in combination with corticosteroids has become more important after liver segmental grafts transplantations. The suppression of regeneration could cause risks after liver transplantation especially in small volume grafts. On the other hand, this effect is thought to be useful in transplantations for hepatocellular carcinoma.

Sirolimus Ve Takrolimus’un Karaciğer Yenilenmesine Etkisi

GİRİŞ ve AMAÇ: Bu çalışmada parsiyel hepatektomi yapılan farelerde sirolimus ve takrolimusun, kortikosteroid uygulandığında ve uygulanmadığı durumda, karaciğer yenilenmesine olan etkileri araştırıldı.
YÖNTEM ve GEREÇLER: Bu çalışmada ağırlıkları 200-300 gr arasında değişen 40 adet erkek fare üzerinde gerçekleştirildi. Sirolimus ve takrolimusun, karaciğer yenilenmesine olan etkileri, korkikosteroidlerin ise bu etkiler üzerindeki yeri araştırılmıştır. Parsiyel hepatektomi sonrası 4 gün süreyle sirolimus ve takrolimus tek başına ya da kortikosteroid (prednisolon) ile birlikte verilerek intraperitoneal olarak uygulandı. 4 gün sonunda farelerin karaciğer dokuları tartıldı ve ağırlık indeksi oluşturuldu. Karaciğer dokuları hematoksilen eosin ile boyanıp mitoz açısından incelendi.
BULGULAR: Bu çalışmada takrolimus uygulanan grup ile karşılaştırıldığında sirolimus uygulanan grupta mitotik indeks ve ağırlık indeksi açısından sirolimus aleyhine istatistiksel olarak anlamlı fark saptandı. Sirolimus ile kontrol grubunun karşılaştırılmasında ise her iki indeks açısından da anlamlı fark saptanmazken sirolimus ve steroidin beraber kullanıldığı grubun verilerinin kontrol grubu ile karşılaştırılması sonucunda mitotik indeks açısından sirolimus ve steroid aleyhine istatistiksel olarak anlamlı fark saptandı.
TARTIŞMA ve SONUÇ: Sirolimusun karaciğer yenilenmesini baskılayıcı etkisinin kortikosteroidler ile ne yönde değiştiği özellikle segmental greftlerle yapılan karaciğer transplantasyonu sonrasında önem kazanmaktadır. Karaciğerdeki yenilenmenin baskılanması küçük hacimli parsiyel nakillerde risk oluşturabilir. Öte yandan bu etkinin hepatosellüler kanser için yapılan transplantasyonlarda faydalı olacağı düşünülmektedir.
