The effect of preoperative oral care on peristomal infection in patients undergoing percutaneous endoscopic gastrostomy tube

INTRODUCTION: PEG administration has many major and minor complications such as gastrointestinal perforation, bleeding, tube blockage, aspiration pneumonia, peristomal infection, necrotizing fasciitis. Peristomal infection is one of the most common minor complications. In this study, we aimed to investigate the effect of oral care on peristomal infection before PEG procedure.

METHODS: The patients were divided into two groups as oral care group (Group 1) and untreated (Group 2) before PEG procedure. Peristomal infections were compared between Group 1 and Group 2 during 30 days after PEG administration. The data were recorded in SPSS 22 version. Statistical comparisons were performed using chi-square test. P <0.05 was accepted as the level of significance.

RESULTS: A total of 146 patients were included in the study, 74 patients who underwent oral care (Grup 1) and 72 patients without oral care(Grup 2). Peristomal infection was found in 17 (23.5%) of 72 patients who had no oral care prior to PEG. Peristomal infection was found in 8 (10.8%) of 74 patients who underwent oral care before PEG. Peristomal infection was significantly lower in patients who underwent oral care prior to PEG treatment (P-0,04).

DISCUSSION AND CONCLUSION: The disadvantage of our study is that it is retrospective. With the broader prospective studies, the accuracy of our results can be confirmed. In conclusion, we recommend oral care before the procedure in order to reduce peristomal infection.

Perkütan endoskopik gastrostomi tüpü uygulanan hastalarda işlem öncesi ağız bakımı yapılmasının peristomal infeksiyon üzerine etkisi

GİRİŞ ve AMAÇ: GİRİŞ ve AMAÇ: PEG uygulamasının gastrointestinal sistem perforasyonu, kanama, tüp tıkanması, aspirasyon pnömonisi, peristomal infeksiyon, nekrotizan fasiit gibi bir çok major ve minor komlikasyonları vardır. Peristomal infeksiyon en sık görülen minör komplikasyonlardan birisidir. Bu çalışmada amacımız PEG işlemi öncesi uygulanacak ağız bakımı yönteminin peristomal infeksiyonu üzerine etkisini araştırmaktır.

YÖNTEM ve GEREÇLER: Olgular PEG işlemi uygulanmadan önce ağız bakımı yapılanlar (Grup 1) ve ağız bakımı yapılmayan (Grup 2) olarak iki gruba ayrıldı. Grup 1 ve Grup 2 arasında PEG uygulamasından sonra 30 günlük süre içinde ortaya çıkan peristomal enfeksiyonlar karşılaştırıldı. Veriler SPSS 22 versiyonuna kayıt edildi. İstatistiksel karşılaştırmalar ki-kare testi kullanılarak yapıldı. P<0,05 anlamlılık düzeyi olarak kabul edi

BULGULAR: İşlemden önce ağız bakımı uygulanan Grup 1’de 74 ve ağız bakımı yapılmayan Grup 2’de 72 olmak üzere 146 hasta çalışmaya dahil edildi. PEG uygulaması öncesi ağız bakımı yapılmayan (Grup 2) 72 hastanın 17’sinde (% 23,5) peristomal infeksiyon saptandı. PEG uygulaması öncesi ağız bakımı yapılan (Grup 1) 74 hastanın ise 8’inde (% 10,8) peristomal infeksiyon saptandı. PEG uygulaması öncesi ağız bakımı yapılan hastalarda yapılmayanlara göre peristomal infeksiyon anlamlı düzeyde daha az görüldüğü ortaya çıkmıştır (P=0,04).

TARTIŞMA ve SONUÇ: Çalışmamızın retrospektif olması dezavantajıdır. Prospektif yapılacak daha geniş çalışmalar ile bizim sonucumuzun doğruluğu teyit edilebilir. Sonuç olarak biz PEG uygulanacak olgulara, peristomal enfeksiyonu azaltmak için, işlem öncesi ağız bakımı yapılmasını öneriyoruz.
