FAST HUGS WITH ICU Efficacy in Intensive Care Patients

INTRODUCTION: In order to regularly follow all of the vital parameters without overlooking any of them during the follow-up of patients in the Intensive Care Units, abbreviations formed by combining the initial letters were utilized. Our aim is to evaluate the control parameters, and by using "FAST HUGS WİTH ICU", to assess the potential deficiencies that can be identified as a whole during the daily follow-ups.
METHODS: This study was performed during the random daily follow up of 29 level 3 and 24 level 2 intensive care patients that were followed up outside the Anesthesia Intensive Care Units of University of Health Sciences İzmir Tepecik Research and Training Hospital and SuatSeren Research and Training Hospital. Each control parameter was evaluated together with the responsible doctor and the detected implementation deficiencies were recorded.
RESULTS: The mean age of the participating patients was 65.53±14.71, and their median weight was 75 kg. It was found that sufficient calorie intake could not be provided, there were treatment deficiencies in sedation, analgesia and thromboprophylaxis applications, daily research and treatment plans of 11 patients were not recorded, daily medical treatments of 16 patients were not reviewed, CAM-ICU scoring was not performed for any of the patients, and there were also deficiencies in follow ups as well as applications for all other parameters. It was found that hourly urine follow-up was not performed for 12 patients with foley catheter, NG was applied to 4 patients for longer than 4 weeks and not replaced with PEG. It was noted that there were defiencies in follow-up infection markers
DISCUSSION AND CONCLUSION: Concordant with the literature,we found that significant deficiencies can occur in daily follow up of patients. This mnemonic is a parameter where 15 parameters are simultaneously evaluated, and providing very fast and collective review in the hands of experienced users.

Yoğun Bakım Hastalarında FAST HUGS WITH ICU Etkinliği

GİRİŞ ve AMAÇ: Yoğun Bakım Ünitelerinde hastaların takipleri sırasında tüm hayati parametrelerin atlanmadan düzenle takip edilmesi için baş harflerin birleştirilmesi ile oluşturulan kısaltmalar kullanılmıştır. Amacımız kontrol parametrelerini gözden geçirerek “FAST HUGS WITH ICU” ile günlük takiplerde saptanabilecek olası eksikliklerin topluca değerlendirilmesidir.
YÖNTEM ve GEREÇLER: Bu çalışma; Sağlık Bilimleri Üniversitesi İzmir Tepecik EAH ve Suat Seren EAH’lerinde Anestezi Yoğun Bakım Ünitesi dışında takip edilen; 29’u üçüncü düzey ve 24’ü ikinci düzey yoğun bakım hastasının rastgele bir günlük takibi sırasında uygulanmıştır. Her bir kontrol parametresi takip eden hekim ile birlikte değerlendirilmiş ve tespit edilen uygulama eksiklikleri kayıt edilmiştir.
BULGULAR: Yaş ortalaması 65.53±14.71, ağırlık ortancası 75 kg (65-80)’di. Çalışmamızda özellikle gerekli kalori
Çalışmamızda yeterli kalori alımının sağlanamadığı, sedasyon, analjezi ve tromboproflaksi uygulamalarında tedavi eksiklikleri olduğu, 11 hastada günlük araştırmaların ve tedavi planlamasının kayıt edilmediği, 16 hastada günlük medikal tedavilerin gözden geçirilmediği, hastaların hiçbirinde CAM-ICU skorlamasının yapılmadığı ve diğer tüm parametreler için de takiplerde ve uygulamalarda eksiklikler olduğu saptanmıştır. 12 foley sondalı hastada saatlik idrar takibi yapılmadığı, 4 hastada 4 haftadan uzun süredir NG uygulandığı ve PEG’e geçilmediği saptanmıştır. Hastaların enfeksiyon markırlarının takibinde eksikler olduğu kaydedilmiştir.

TARTIŞMA ve SONUÇ: Çalışmamızda literatür ile uyumlu olarak günlük hasta takibinde önemli eksikliklerin yapılabileceğini gördük. Bu nimonik 15 parametrenin aynı anda değerlendirildiği ve deneyimli kullanıcıların elinde çok hızlı ve kolektif bir gözden geçirme sağlayan bir parametredir.
